Schedule an Initial Assessment to get started. After you make your appointment, you will receive an email with a link to the patient portal to complete the intake process.

The following documentation must be provided at least 48 hours before your initial assessment:

All intake forms completed in the patient portal

Insurance referrals (if required)

Labs

Medical records

If not received within 48 hours, we will have to open the appointment time for other clients.

A credit card is required to hold your appointment.

At least 48 hours notice is required to cancel or reschedule an appointment. Your credit card will be charged $100 in case of late cancellation or no show (exceptions granted due to illness).

Pediatric Clients: If making an appointment for your child, please schedule it in his/her name (not your own).​ We can only bill insurance if the child is in attendance.

Aetna clients: Aetna requires that my services be "prescribed by a physician". Therefore, prior to your session, we must receive a referral from your physician that includes your ICD-10 diagnosis code(s). Sessions limited to 55 minutes unless you want to self-pay for the extra time. More coverage info here.
